• You must view the schedule below before submitting this form to choose the correct class to change to.

• The student MUST meet the age & experience criteria for the new class you are requesting.

• The class must NOT be showing as full / waiting list.

• Students performing in the annual showcase are not permitted to change class in the Winter Term.

• This is a request form and does not confirm your place in the new class. You must wait for confirmation from the office.
